The HTTP /mcp endpoint now requires credentials by default; set MCP_AUTH_ENFORCEMENT=observe to keep anonymous access and record authentication migration events, while /mcp/oauth and Claude Code plugin requests keep their challenge in both modes. Expired or revoked OAuth access tokens now receive an HTTP 401 with a WWW-Authenticate invalid_token challenge so clients refresh instead of showing a tool error, with MCP_OAUTH_TOKEN_VALIDATION=off to disable the check.
