Large PR file limits

Automatic PR review file limits now apply by plan. Pro reviews are limited to 150 files, while Pro+ reviews are limited to 300 files. The overall ceiling remains 300 files.

Eligible non-Enterprise users on GitHub and GitHub Enterprise Server can select Review on demand using usage pricing to continue a review that exceeds its tier limit without exceeding the 300-file ceiling. The action reuses CodeRabbit's existing usage-based billing and safeguards.
